DeLand Foot & Leg Center is a patient-centered, state-of-the-art practice led by a highly trained and experienced surgeon who specializes in wound care and limb salvage of the foot and ankle with an emphasis on education, health and wellness.
In addition to our foot and ankle services, we offer a curated menu of spa services. Our spa takes a holistic approach to pampering while providing the highest quality medical grade treatments and sterilization.
From medical and surgical treatment options to aesthetics and wellness, our clinic and spa offer a well-rounded and comprehensive approach for women, men, children and seniors. We strive to exceed patient expectations by fostering an environment of excellence, hospitality and compassion.
At Dr. Pulapaka’s office, we understand that you will have financial questions. Our goal is to provide options that will fit your budget comfortably. Aside from accepting most major medical insurances and Medicare we also offer self-pay services.
Our experienced staff can give you full-service assistance with your insurance and written estimates. There are low monthly payments and many choices so that you can tailor your financing to suit your needs.
We accept all major credit cards and all co-payments are due the same day of service. For your convenience, we accept credit and debit cards. To keep our costs down and offer the most competitive rates, a 3.6% convenience fee is applied to all credit and debit card transactions.
If you prefer to avoid this fee, we are happy to accept cash or check for your payment.
Prior to your first visit, you can print all three of your forms to review and complete at home.
We DO NOT OVERBOOK, all new patients are scheduled for 1hr of appointment time and returning patients, a minimum of 30min. Therefore, we have a strict cancellation policy. Currently, it takes about 4 weeks for a regular appointment.
CANCELLATION POLICY: DeLand Foot and Leg Center, LLC requires a 48 business hours cancellation notice. We ask your full cooperation not to burden you. If your appointment is on Monday morning you can call Saturday to cancel. You may leave a message with our service when calling the office after hours. A $75.00 fee applies for appointment cancellations in less than 48 business hours, a $75 fee for canceled vascular studies, and a $250.00 fee applies for surgery canceled in less than 72 hours.
Prescription Refill requests take 48 business hours to process, so please allow adequate time for us to fill your request. Disability, Insurance, and/or Medical Forms requests usually take 5-7 days to complete after the $35.00 processing fee has been paid for each packet, this includes a minimal verification of the chart documents. If you require documentation that is beyond that scope, it is pre-billed at $750/hr. Lab results: it is the patient’s responsibility to follow through on their care and request a copy or the results of any and all labs or testing performed in the office or out of the office, i.e. MRI, Bone Scan, and Pathology.

Wound Care
- We need all prior medical records, including oral & topical antibiotics, IV antibiotics, blood work, culture testing, and progress notes. If you have questions call (386) 738-3733
Orthopedics
- We will need records from all physicians that have treated this issue in the past, whether the results were good or bad, we need to know about injection locations, surgeries, physical therapy.
- Dr. Pulapaka does not do ELECTIVE SURGERY. Her Speciality is osteomyelitis, bone infection and the associated surgeries.

Vascular
- We need a copy of your last Cardiology/Vascular Surgeon Progress Note; and any studies they have completed, including any heart or vascular surgeries. If you have additional studies or vascular reports, please contact the office for instructions. (386) 738-3733
Dermatology
- Bring a copy or fax over your last pathology findings and photographs. FAX: (386) 738-3733
Neurological
- We will need any spinal imaging and/or nerve studies that have been performed.
- Pain diaries are important in determining your treatment options, so bring it with you to the exam or call for one (386) 738-3733
Routine Nail & Callus Care
- Medicare and Commercial insurance have strict criteria for covering ROUTINE NAIL & CALLUS CARE. Therefore, we will need your past podiatrist’s records to help with the approval process or we may require additional testing to determine your medical needs. Pediatrics
- We have a variety of orthotics for children that are affordable and convenient .
- Developmental history is important with a pediatric exam, do not forget to let us know about any pregnancy complications or delivery issues.
