Discontinuation of  Routine Nail & Callus Trimming Services

Effective December 31, 2023, our practice will no longer offer routine foot care services (without a concurrent, medically necessary problem addressed in the same visit) for Medicare and Commercial beneficiaries. This transition, for which we have been preparing patients since June 2023, reflects our increasing focus on providing highly specialized Podiatric Lifestyle Medicine treatment plans for complex conditions. The demand for these advanced care pathways has grown significantly, requiring our full attention and resources.

While we understand the importance of routine foot care, numerous other skilled providers in our area are well-equipped to meet these needs. Our expertise and capacity are now primarily dedicated to patients requiring comprehensive and intricate treatment strategies. We will continue to offer nail testing and analysis to guide the development of appropriate treatment plans utilizing topical or oral medications when medically indicated.

This decision allows us to concentrate on delivering the highest level of care for complex podiatric issues, ensuring that those with the most challenging conditions receive the specialized attention they require. We appreciate your understanding as we adapt our services to best serve the evolving needs of our patient population and the wider medical community.

Medicare & Medicare Replacement Guidelines for Routine Foot Care

Medicare and Commercial Insurance
Medicare law and some commercial insurances exclude from coverage routine foot care, routine physical checkups, and services that are not reasonable and necessary for the diagnosis or treatment of an illness or injury or to improve the functioning of a malformed body member. Therefore, these services are precluded from coverage even though they may be within your Podiatric Physician’s scope of practice under State law.

ABN Form is used when Medicare payment is expected to be denied. The ABN is issued to transfer potential financial liability to the patient.

What Is an Ingrown Toenail?

When a toenail is ingrown, it is curved and grows into the skin, usually at the nail borders (the sides of the nail). This “digging in” of the nail irritates the skin, often creating pain, redness, swelling, and warmth in the toe.
If an ingrown nail causes a break in the skin, bacteria may enter and cause an infection in the area, which is often marked by drainage and a foul odor. However, even if the toe isn’t painful, red, swollen, or warm, a nail that curves downward into the skin can progress to an infection.

Sometimes, she will remove the offending nail. After applying a local anesthetic, the doctor removes part of the nail’s side border. Some nails may become ingrown again, requiring removal of the nail root.
Following the nail procedure, a light bandage will be applied. Most people experience very little pain after surgery and may resume normal activity the next day. Please see our post-surgery nail instructions.

We have a 48-hour guarantee for ingrown nail care or surgery.

This ingrown had a piece of nail that the patient could not cut himself in the corner, which was coming out the tip of the toe.

Topical Treatment Options, when oral medication is not an option

Tailored Topical Power: We offer innovative topical solutions like Tolcylen, a powerful, office-dispensed option. Featuring a patent-pending formula combining FDA-approved antifungals with multiple cosmetic agents, it delivers potent action right where you need it.

Maximize Effectiveness: Before applying, cleanse thoroughly under your nails using a toothbrush and Peroxide, followed by mild soap and water. This routine removes debris and optimizes treatment penetration.

Beyond the Topical: Remember, topical treatments often work best in conjunction with oral medications. Our holistic approach includes daily cleaning and potential oral options for comprehensive healing.

Protection + Prevention with key medical products, like SokTek

Our socks are constructed with two products that are designed to maximize yarn technology and accomplish product sustainability.  REPREVE® Recycled Polyester is one of the most certified, traceable fibers available in the world. High-quality, recycled fibers are made from 100% recycled materials, including post-consumer water bottles and pre-consumer waste.  This yarn incorporates SORBTEK® for moisture management, which makes it ideal for our socks.

The natural antimicrobial effect of bamboo does not allow the growth of microbes, where bamboo actually repels the colonization of E. coli and S. aureus.  The bactericide characteristics are mainly due to the presence of a bio-agent “bamboo Kun” in the fiber, which bonds tightly with bamboo cellulose molecules.  The growth rate of micro-organisms on 100% cotton is the highest.

Under development, Coats Protect+ spun yarns are an inherent antimicrobial yarn solution built into product DNA, inhibiting the growth of microbes in or on fabrics while providing all-day freshness. This is an antimicrobial yarn that provides a sustainable option for antimicrobial requirements in at-risk employees.

Athletes foot, Cellulitis, Warts

After examining the toe, Dr. Pulapaka will select the treatment best suited for you. If an infection is present, an oral antibiotic may be prescribed.
Sometimes a minor surgical procedure, often performed in the office, will ease the pain and remove the lesions or biopsy a growth.  If Dr. Pulapaka has prescribed an oral antibiotic, be sure to take all the medication, even if your symptoms have improved.
