Some foot care procedures are generally considered to be routine and may not be covered; therefore, The Advance Beneficiary Notice of Noncoverage (ABN) must be completed in these instances:

  • Nails: Fungal & Ingrown
  • Corn & Callus Treatments
  • Deformed Nails
  • Detached Nails
  • Discolored Nails

Medicare and Commercial Insurance
Medicare law and some commercial insurances exclude from coverage routine foot care, routine physical checkups, and services that are not reasonable and necessary for the diagnosis or treatment of an illness or injury or to improve the functioning of a malformed body member. Therefore, these services are precluded from coverage even though they may be within your Podiatric Physician’s scope of practice under State law.

ABN Form is used when Medicare payment is expected to be denied. The ABN is issued to transfer potential financial liability to the patient.  

What Is an Ingrown Toenail?

When a toenail is ingrown, it is curved and grows into the skin, usually at the nail borders (the sides of the nail). This “digging in” of the nail irritates the skin, often creating pain, redness, swelling, and warmth in the toe.
If an ingrown nail causes a break in the skin, bacteria may enter and cause an infection in the area, which is often marked by drainage and a foul odor. However, even if the toe isn’t painful, red, swollen, or warm, a nail that curves downward into the skin can progress to an infection.

Sometimes, she will remove the offending nail. After applying a local anesthetic, the doctor removes part of the nail’s side border. Some nails may become ingrown again, requiring removal of the nail root.
Following the nail procedure, a light bandage will be applied. Most people experience very little pain after surgery and may resume normal activity the next day. Please see our post-surgery nail instructions.

This ingrown had a piece of nail that the patient could not cut himself in the corner, which was coming out the tip of the toe.

Discontinuation of  Routine Nail & Callus Trimming Services

With a heavy heart, we must announce the discontinuation of routine foot care services without another medical problem during the same visit for Medicare and Commercial beneficiaries, effective January 1, 2024. We have been preparing our patients for this potential change since June 2023.  We have never been a pedicure office and insurance companies have never provided coverage to routine nail care. The good news is that we will still be providing nail testing and analysis, along with creating a treatment plan of care using topical or oral medications. 

We understand the importance of routine foot care in maintaining the overall health and well-being of our patients, particularly those who rely on Medicare for their medical needs. Unfortunately, due to recent reductions in Medicare reimbursement, I am unable to physically see twice the number of nail and callus patients to make up for the financial shortfall. This decision to discontinue these services was incredibly difficult, as I value the trust you’ve placed in me for your care.

Our socks are constructed with two products that are designed to maximize yarn technology and accomplish product sustainability.  REPREVE® Recycled Polyester is one of the most certified, traceable fibers available in the world. Athletes foot, Cellulitis, Warts

After examining the toe, Dr. Pulapaka will select the treatment best suited for you. If an infection is present, an oral antibiotic may be prescribed.
Sometimes a minor surgical procedure, often performed in the office, will ease the pain and remove the lesions or biopsy a growth.  If Dr. Pulapaka has prescribed an oral antibiotic, be sure to take all the medication, even if your symptoms have improved.
